Prevention Therapist/Case Manager
Company: Walden House, Inc. Job Description: The Health Educator is responsible for direct service provision to consumers. This includes Early Intervention Program (EIP) Presentations, Individual Risk Assessment Counseling Sessions Multiple Session Workshops, Individual Case Management, Group Process, and office support Counseling Responsibilities: Responsible for facilitating no less than three groups weekly and for a minimum of 15 counseling sessions weekly. (Scheduling may require working evenings and weekends.) Responsible for group preparation, creation of appropriate hand-outs, visual aids and maintenance of workshop related supplies. Follows up on linkages made, assess appropriateness of those linkages and ensure that all clients needs have been, or are being met. Proper recording of all linkages made and follow up steps to be taken in client file at each Individual Risk Assessment Counseling Session. Provides links for STD testing for clients: discusses testing and test results with client and accompany client to test facility, if appropriate. Provides sensitive harm reduction counseling, education, and referrals to clients according the U.S. Center for Disease Control and Prevention’s (CDC) HIV testing protocol. Conducts behavioral risk assessments on each client. Provides brief therapy services for prevention-related issues. PREREQUISITES: CANDIDATES MUST BE ABLE TO CLEAR A BACKGROUND CHECK.
