HIV Prevention Health Educator/Case Manager

Job Description:

The Health Educator is responsible for direct service provision to consumers. This includes Early Intervention Program (EIP) Presentations, Individual Risk Assessment Counseling Sessions Multiple Session Workshops, Individual Case Management, Group Process, and office support

Counseling Responsibilities: Responsible for facilitating no less than three groups weekly and for a minimum of 15 counseling sessions weekly. (Scheduling may require working evenings and weekends.) Responsible for group preparation, creation of appropriate hand-outs, visual aids and maintenance of workshop related supplies. Follows up on linkages made, assess appropriateness of those linkages and ensure that all clients needs have been, or are being met. Proper recording of all linkages made and follow up steps to be taken in client file at each Individual Risk Assessment Counseling Session. Provides links for STD testing for clients: discusses testing and test results with client and accompany client to test facility, if appropriate. Provides sensitive harm reduction counseling, education, and referrals to clients according the U.S. Center for Disease Control and Prevention’s (CDC) HIV testing protocol. Conducts behavioral risk assessments on each client.  Provides brief therapy services for prevention-related issues.

Community Responsibilities: Required to attend relevant community based meetings and have knowledge of other Bay Area organizations.

Administrative Responsibilities: Has direct involvement with data collection and entry and assures the confidentiality of all client data. All referrals should be followed-up with the client when reviewing action steps. Referral follow-up and services received must be documented in the PCM plan or in progress notes. Health Educators must follow-up on all referrals with the referring agencies. Complete accurate documentation of client encounter.

 
PREREQUISITES:
• Must be able to clear a background check
• Must have a Bachelor's Degree in a related field or equivalent experience in HIV prevention services.  Experience in addiction treatment services preferred.
• Must have a minimum of two years paid work experience in HIV education or counseling services.
• Must have completed or be enrolled with any of the State of California Department of Alcohol and Drug Programs (ADP) for California Alcohol and Drug Certification (CADC).  New hires not meeting this requirement must register for ADP education within six months of hire.  Certification must be completed within five years of registration.
• Demonstrated ability and/or commitment to working effectively with clients from diverse backgrounds, with sensitivity to ethnic, cultural, gender, and sexual minorities.
• Excellent attention to detail, self-motivation, ability to work independently as well as part of a team, and strong organizational skills.
• Comfortable engaging in open, nonjudgmental conversations with clients regarding their sexual and drug using behaviors.
