Job Description:
The SAP Service Coordinator will be responsible for: development of transitional plans, aftercare marketing and the screening of inmates at the in-prison substance abuse program (SAP) located in the institution. Documentation and ongoing communication with the California Department of Corrections & Rehabilitation personnel.
Case Management Responsibilities: Coordinates the transition into a community-based provider program, monitors program participants, maintains project documents and records. Develop continuum of care plans.
Clearance Requirements: Must undergo a background investigation and criminal records check to obtain clearance to work at criminal justice State institution. Applicant cannot be on probation or parole. Final approval is contingent upon clearance.
PREREQUISITES
• High school diploma or GED; Drug and Alcohol certification with 2 years of paid experience providing substance abuse services to criminal justice population, OR BA degree in one of the health or social sciences with 1 year paid experience providing substance abuse services to criminal justice population
• Valid California driver’s license and a reliable vehicle
• Excellent computer skills including Office 97 (Word, Excel, Power Point).
• Ability to appropriately handle confidential information
• Must have at least 3 years off of Probation or Parole & be able to be cleared by CDCR
BACKGROUND INVESTIGATION:
Potential staff must undergo a background investigation and criminal records check to obtain clearance to work at criminal justice institutions. Must be off of probation or parole for at least three (3) years. Final approval is contingent upon clearance by the CDCR.
