The Outpatient Case Manager I position is responsible for clients during outpatient phases of treatment. In conjunction with specialized staff, develops treatment plans focused on relapse prevention and sustaining drug-free living. Case-Manager Monitors client sobriety within an abstinence model program which also incorporates the Harm Reduction philosophy. Case Manager works with clients on issues they face in areas of living drug-free. Provides Case Management to clients in the areas of Primary Care, Vocational goals, Housing needs and Independent Life Skills. Goal is client completion of Outpatient Program within the most effective time frame.
Treatment Planning: In case conference settings, the case-manager evaluates each client’s clinical/vocational needs and develops treatment plans that coordinate Agency and community resources. Responsibilities include working with specialists to facilitate continued and timely revisions of treatment plans and determine client’s readiness to move to next phase of treatment or discharge.
Caseload Responsibilities: This position holds individual and group counseling sessions with clients in caseload. They provide referrals to outside resources, such as Support Groups, Housing Assistance, Primary Care, etc. They assist clients with guidance in areas of substance abuse, recovery and self-responsibility. They are responsible for facilitating group process, workshops and treatment adherence sessions.
Administration and Compliance: In accordance with guidelines established by Walden House to satisfy internal and external administrative demands, and those of evaluating agencies, maintains accurate records on all clients.
Community Responsibilities: This position leads community meetings and specialized groups, teaches classes, plans clinical days and holidays. They inspect the facility and take responsibility for all clients as unforeseen situations arise. They communicate with primary staff verbally and through staff log. They perform urinalyses, plan completion ceremonies as well as assist in planning Celebrations of Achievement. They also ensure that graduates are connected with the Walden House Alumni Group.
Role Model: This position acts as role model by following norms, demonstrating honesty, integrity and a willingness to listen to client issues, maintaining a neat office and work area, and demonstrating self-responsibility.
PREREQUISITES
• Must be a Certified Alcohol or other Drug Counselor. If not, must be registered for California Alcohol and Drug Certification with any of the State of California Department of Alcohol and Drug Programs (ADP) approved agencies within 6 months of hire. After registration, must obtain certification within 5 years.
• Must have clear understanding of addiction and addictive process
• Must have basic Addiction Counselor skills including Case Management Skills, Group Process, Group Facilitation, Conflict Resolution, Professional Boundaries, Ethics, Judgment and the ability to work effectively with a diverse population
• Strong reading comprehension with ability to analyze complex information. Good
• Writing skills with proper grammar, good spelling, and good handwriting.

COMPENSATION: $30,000 - $32,000 plus Walden House offers a full range of benefits, including medical, dental and vision, as well as 401(A) & 403(B) for all Full time employees.
