Job Description:

This position works with high risk, court-involved youth, who often are dealing with co-occurring disorders. The position learns a number of standardized interventions, and works with clients and their families both while in residential treatment and after completing residential treatment. The position works essentially as a “re-entry” counselor focusing on goals of family reunification, education, sobriety, employment, and community evolvement. Part of the job involves identifying resources in the client’s community and coordinating successful linkages with those resources. The position will meet with clients and families in the home after the client leaves residential treatment. Service, generally in the field, includes community based advocacy, assessment, case management, counseling, education, and resiliency building and strengthening of family support system. This position involves delivery of evidence-based interventions to youth and parent/caregiver(s). Building life skills, resiliency, communication, and boundaries are primary responsibilities.

Clinical: Provides clinical, direct service intervention planning and provision with persons served and their significant others.  Services provided include advocacy, assessment, case management, counseling, education, and reunification.
Documentation: Documents service and treatment provision per established policies, procedures, and standards of care.
Linkages & Referrals: Utilizes the clinical team and community resources for progressive case consultation, management, and ensures optimal service.
 
 
PREREQUISITES
Education: Position requires a Graduate Degree or licensure in the State of California (MSW, LCSW, LMFT, Ph.D.)
Knowledge: Training in case management, family systems theory, and/or recovery / wellness modality treatments.
Skills: Ability to interact with diverse populations around sensitive topics regarding behavioral health issues, especially addictive and psychiatric disorders. Specifically, experience working with youth and their significant others on behavioral health issues.
